DBS Status Check Authorisation Form

To be completed by volunteers who have completed a DBS check through the University of Warwick and subscribed to the DBS Update Service.


 
In addition to a valid enhanced DBS check the University requires a non-UK criminal record check (or ‘Certificate of Good Conduct’) to be provided if you have lived overseas within the last five years (for a period of six months or more).

If your DBS check was not processed by Warwick Volunteers you will need to bring your disclosure certificate and University card to the Warwick Volunteers office for us to check.

By selecting YES below, I understand that I am providing authorisation for Warwick Volunteers to carry out a Status Check through the DBS Update Service as frequently as may be required for the duration of my time at Warwick Volunteers, until such time as I notify Warwick Volunteers that I no longer occupy a role requiring the DBS Certificate. The University of Warwick will process the information submitted to carry out a Status Check through the DBS Update Service for the purposes of facilitating your chosen volunteering project(s). The legal basis for processing this personal data is that it is necessary for your participation in a volunteering project which requires Safeguarding checks, and consent, if you have ticked the box. Your personal data will not be shared or disclosed to any third parties external to the University of Warwick except project leaders (who may be other students or external organisations) for the purpose of running your chosen volunteer project(s). Your personal data will not be transferred outside of the EEA, will be kept securely by the University of Warwick and will be retained for a period of 7 years. The University of Warwick is the Data Controller of this information and is committed to protecting the rights of individuals in line with Data Protection Legislation.
