Add a phone number field to a form

Use the phone number field for respondents to enter their phone number in a form.
  1. Go to the form you wish to edit.
  2. Go to Edit > Edit formsbuilder page.
  3. Under Add a text box, select Phone number field.
  4. In the Field name box, enter the name you want to use as the column heading for this field in the form submissions table. For example:

    Telephone
  5. In the Label box, enter the instructions to show the respondent on the form – for example:

    Your telephone number

    If you don't specify a Label, the Field name displays next to the box instead.

  6. Choose a Width of the phone number box as a percentage of the page width. For example, 100% spans the entire page.
  7. Optionally, in the Prefilled text box, enter a hint or example answer to help the respondent understand the information you're asking for. This text appears in the text box when the form loads. For example, if you require international dialling codes:

    Include international dialing code – for example +44 24 7123 4567
  8. Choose whether to display the Label above the text box or at the left. The Label displays at the left by default.
  9. To make the phone number field mandatory – the respondent must enter a number before they can submit the form – select the Required checkbox.
  10. To show the phone number field only to editors of submissions – and hide it from respondents – select Only visible to editors.
  11. Select Save.
  12. As you're collecting personal data, ensure your privacy notice accurately reflects the information you're collecting and what you intend to do with it.
